개요
  • 결핵 코호트는 2016년부터 운영되어 오며 결핵의 발병 및 치료 경과 등에 영향을 미치는 요인을 규명하고, 보다 빠른 진단과 치료 향상에 필요한 과학적 근거들을 축적하여 국가 결핵 관리 정책 수립에 기여하고 있습니다.
  • 최근에는 고령 결핵 증가 등 환경 변화와 환자 중심 치료의 인식 전환에 발맞추어 연구 대상과 범위를 확대하고 있습니다. 또한 고품질의 임상 정보와 생물자원을 지속적으로 수집·관리함과 동시에 결핵 퇴치 기술 개발 연구에 활용하고 있습니다.
추진 경과
  • 결핵 코호트 기반 구축(2016~2018)
    • 결핵 환자를 체계적으로 등록하고 관리할 수 있도록 표준화된 운영체계를 마련하였습니다. 전문적인 조사를 위해 ▲임상적 평가(8종), ▲사회경제적 지위(6종), ▲실험실 평가(13종), ▲결핵정보(3종)을 포함한 증례기록서(CRF)를 개발함으로써, 다양한 지표를 수집하고 분석할 수 있는 기반을 갖추었습니다.
    • 이를 통해 2개 병원을 중심으로 만 18세 이상 활동성 결핵 환자 600명을 등록하고, 결핵균, 객담 등 1,000건 이상의 생물자원을 확보하여 결핵 코호트 연구의 기반을 마련하였습니다.
  • 결핵 코호트 고도화: COSMOTB 운영(2019-2026)
    • 2019년부터 참여병원을 20개로 확대하여 결핵 환자 코호트 연구(COSMOTB)를 본격화하였습니다.
    • 웹 기반 전자증례기록(eCRF) 시스템을 도입하여 데이터 관리의 신뢰성을 높였으며, ▲임상적 평가(12종), ▲사회경제적 지위(6종), ▲실험실 평가(12종), ▲결핵정보(4종)으로 조사항목을 확대 및 정비하였습니다.
    • 2019~2024년 동안 2,284명 환자를 추가 등록하여 누적 2,884명의 코호트를 구성하였고, 치료 종료 이후에도 1년 이상의 추적 관찰로 치료 결과와 재발 여부를 확인하고 있습니다.
    • 또한, 결핵균과 객담 외에도 혈액(혈장, 혈청), 뇨 등으로 자원 종류를 확대하여, 5,000건 이상의 생물자원을 추가 수집하였고, 더불어 임상 정보에 대한 데이터 정제를 수행하여 데이터의 품질과 활용도를 높였습니다.
    • 축적된 임상정보 및 생물자원을 활용하여 내성결핵 치료 향상을 위한 다양한 연구를 수행하였으며, 그 결과는 결핵진료지침(5판)에 반영되었습니다.
  • 연구대상 확대: 무증상 결핵 코호트(2026~)
    • 2026년부터는 결핵 환자를 증상 유무에 따라 체계적으로 구분하고, 무증상 환자를 초기 단계부터 선별․등록하는 코호트를 운영하고 있습니다.
    • 결핵 환자(무증상+유증상)과 함께 이들의 가족 접촉자를 포함하여 총 1,400명 규모로 모집을 목표로 하고 있습니다. 가족 접촉자에 대해서는 1년 이상 발병 유무를 관찰하여 결핵의 전파 양상을 분석할 계획입니다. 또한, 기존 검체와 더불어 혈액 기반 면역세포, 구강 유래 검체 등으로 수집 자원을 다양화하고 있습니다.
    • 무증상 결핵의 연구 결과는 향후 맞춤형 결핵 관리 전략 수립에 활용하고자 합니다.
